Try using sprint PCB and associated software to convert pdf to pcb file and then export your Gerber.
Search for Sprint PCB in Sonsivri forums and you will find it

convert the PDF file to a GERBER file using Gerbview
 the resulting GERBER file into Sprint Layout which allow snapping to  ...
 center of a pad
 try to guess the diameter of each pad on the  ...
 with the right hole size
export the Excellon file.

FAB 3000 is a software that do this. Is a software that can do this. If it is a one time job. Perhaps you can use the trail version for this
