As far as I know there is no such a program.

But you can do the following:
- convert the PDF file to a GERBER file using Gerbview
- import the resulting GERBER file into Sprint Layout which allow snapping to the center of a pad
- try to guess the diameter of each pad on the Gerber file and place over each pad from Gerber file a pad with the right hole size
- export the Excellon file.

The reason for recommending the Sprint Layout software is that in this software it is much easier to place a pad over another and have them aligned (due of the feature of the Sprint Layout that allow snapping to the center of a pad).
